The long-running discourse on respect for human dignity and autonomy in the physician-patient relationship pertaining to persons with dementia (PwDs) is explored deeply in this paper through the use of a real-life case, to highlight the complex interplay between autonomy and best interest when it comes to a PwD's experiential and critical interests. Many scenarios and perspectives are described and applies to the case. However, there are a few perspectives, which are touched upon that could do with further scrutiny. Firstly, (...) it has been argued that rigid adherence to the advance directives of PwDs means that PwDs will be the only people who are not allowed to ever change their minds. This withdrawal of a fundamental freedom hardly accords with respect for human dignity, or for any residual autonomy that these individuals may still enjoy. Secondly, viewing this case through the lens of care ethics, and incorporating the recently proposed conceptual framework of anthropological vulnerability, may reveal valuable insights regarding care provision and caring physician-patient relationships. These perspectives would analyse the role of relational ontology, particularly in relation to significant others, significant communities, and caregivers. (shrink)

In face of the multiple controversies surrounding the DSM process in general and the development of DSM-5 in particular, we have organized a discussion around what we consider six essential questions in further work on the DSM. The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM – whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part 1 of this article took up the first two questions. Part 2 took up the second two questions. Part 3 now deals with Questions 5 & 6. Question 5 confronts the issue of utility, whether the manual design of DSM-III and IV favors clinicians or researchers, and what that means for DSM-5. Our final question, Question 6, takes up a concluding issue, whether the acknowledged problems with the earlier DSMs warrants a significant overhaul of DSM-5 and future manuals. As one might expect of a book of this modest size which bears such a title, this work concentrates upon only one of the many topics subsumed under the rubric "supernatural." After some reflection, the impression develops that this focus is on a colloquy between scholastic and contemporary formulations of the personal relationship between God and man in the process of divinization by grace. Kenny seems to offer the theory of "created actuation by Uncreated Act" as a particularly valuable model (...) of explanation for this personal union. The genre of this work is too technical in its language to be described as popular and perhaps too brief and unnuanced to appeal to a technical audience. The mode under which Kenny approaches his problem is historical. Nonetheless, the author excuses himself from treating antecedent formulations, although he averts to the different way in which the question has been posed in Eastern Christianity. Kenny opens his own account with a brief precis of some "constants" in Catholic grace-doctrine. It is regrettable that William of St.-Thierry and Peter Lombard were not treated more extensively, for the formulation of deifying grace in terms of its "uncreatedness" received a new slant in their works. This slant can be illuminated considerably if one recalls that these theologians had at their disposal only an impoverished, "Platonizing" schema of causal explanation, plotted only on the axis of formal causality and its intentional-exemplar modes. Lacking such a preface in twelfth-century formulations, the author’s treatment of "high Scholastic" grace-doctrine largely from the perspective of Thomas’ and Bonaventure’s insistence on "created grace" seems to be rather abruptly introduced. However, this sense of abruptness disappears if one correctly grasps the thesis toward which Kenny’s argument is aimed. Chapter Four treats the fourteenth- and fifteenth-century nominalist formulations, with Ockham and Biel as examples. Kenny is highly critical of the subversion of the doctrine of grace as intrinsic divinization and as personal union which these theologians effected. Chapter Five is billed as a survey of the theme of supernatural union in post-Tridentine theology. Curiously, the bulk of the chapter is expended on an amicable presentation of the De la Taille-Rahner explanation of graced divinization in terms of quasi-formal causality. Oppenheim is, so to speak an unreconstructed reconstructionist, maintaining the ideal of a relatively perspicuous, non-normative social scientific language-"neither wholly reportative nor merely stipulative, but explicative" --against the tide of contemporary criticisms of "positivism" in this domain. While denying that he himself is a positivist in the sense employed by the critics, he defends a number of the ideas that have often been associated with that term in the philosophy of the social sciences, in particular that of the separability of (...) facts from values. In doing so, he exhibits a clear awareness of the claims of many of his opponents--such writers as Alisdair MacIntyre, Charles Taylor, and Peter Winch. (shrink)

Recovering Reason: Essays in Honor of Thomas L. Pangle is a collection of essays composed by students and friends of Thomas L. Pangle to honor his seminal work and outstanding guidance in the study of political philosophy. These essays examine both Socrates' and modern political philosophers' attempts to answer the question of the right life for human beings, as those attempts are introduced and elaborated in the work of thinkers from Homer and Thucydides to Nietzsche and Charles Taylor.

The authors present an approach to conceptualising and predicting environmental conflicts in which conflicts are analysed as a continuum of disagreement over values and options. They also operationalise this approach using an online values-centred survey tool, the ‘public-to-public decision support system’ (P2P-DSS). The authors put values and conflict in environmental management into perspective. Next, they review how values are defined in scholarship and operationalised for decision support. The relevance of values research to con-flict management is presented. With reference to a (...) real-world aggregate-mining conflict, the authors demonstrate how P2P-DSS can be used to collect data and categorise conflicts to enhance environmental management decision-making. The authors argue that P2P-DSS has potential to support values-sensitive thinking for environmental conflict management. They then set out research priorities to investigate the theoretical and practical implications of this approach. This work contributes to advancing values research in environmental conflict management and expanding values-based decision-making. (shrink)

In the conclusion to this multi-part article I first review the discussions carried out around the six essential questions in psychiatric diagnosis – the position taken by Allen Frances on each question, the commentaries on the respective question along with Frances’ responses to the commentaries, and my own view of the multiple discussions. In this review I emphasize that the core question is the first – what is the nature of psychiatric illness – and that in some manner all further (...) questions follow from the first. Following this review I attempt to move the discussion forward, addressing the first question from the perspectives of natural kind analysis and complexity analysis. This reflection leads toward a view of psychiatric disorders – and future nosologies – as far more complex and uncertain than we have imagined. (shrink)

This interdisciplinary collection of essays highlights the relevance of Buddhist doctrine and practice to issues of globalization. From philosophical, religious, historical, and political perspectives, the authors show that Buddhism—arguably the world’s first transnational religion—is a rich resource for navigating todays interconnected world.

Included in the acute response of lowlanders exposed to reduced oxygen availability is an elevated red blood cell count due to increased erythropoietin (Epo) synthesis. According to current thinking, hypoxia is “sensed” by hydroxylases that permit Hypoxia Inducible Factor 1α (HIF‐1α) to complex with HIF‐1β to form a transcriptional activator (HIF‐1) that drives expression of hypoxia‐sensitive genes (such as EPO) under hypoxic conditions. In altitude‐adapted Andean natives, the Epo hypoxic response may be blunted; however, our data indicate that the DNA (...) sequences of the genes encoding Epo (including the 3′ regulatory region) and HIF‐1α appear to be conserved. Hence, adaptive changes in the Andean hypoxic response are not a consequence of changes in the primary sequence of these proteins or of known transcriptional regulatory domains of EPO. These results suggest that the altered erthropoietic response in Andean natives reflects adaptations in hypoxia sensing, rather than hypoxia response, mechanisms. With this issue we initiate the policy of expanding the scope of Tulane Studies in Philosophy to include, in addition to the work of members of the department, contributions from philosophers who have earned advanced degrees from Tulane and who are now teaching in other colleges and universities. The Editor THE LOGIC OF OUR LANGUAGE ROBERT L. ARRINGTON Wittgenstein wrote in the Tractatus that "logic is not a body of doctrine, but a mirror-image of the world. " 1 In line (...) with his suggestion that a proposition is a 'picture', Wittgenstein argued that propositions 'show' the logical structure of the real. He was insistent, however, that "the apparent logical form of a proposition need not be its real one. " 2 As a result of this we can misunderstand the structure of fact. Philosophical problems arise just when "the logic of our language is mis understood. " 3 It is common knowledge that much of this view of logic was rejected by Wittgenstein himself in the Philosophical Investi gations. There we are told that language has no ideal or sublime 4 logic which mirrors the structure of the extra-linguistic world. Consequently, inferences from the structure of language to the structure of that extra-linguistic world are invalid. Reality can be 'cut up' in any of a number of ways by language. Wittgenstein adopted a view of philosophy which would render that discipline a non-explanatory, non-critical study of the multiple ways in which language can be used. (shrink)

The Monographs produced by the International Agency for Research on Cancer (IARC) apply rigorous procedures for the scientific review and evaluation of carcinogenic hazards by independent experts. The Preamble to the IARC Monographs, which outlines these procedures, was updated in 2019, following recommendations of a 2018 expert Advisory Group. This article presents the key features of the updated Preamble, a major milestone that will enable IARC to take advantage of recent scientific and procedural advances made during the 12 years since (...) the last Preamble amendments. The updated Preamble formalizes important developments already being pioneered in the Monographs Programme. These developments were taken forward in a clarified and strengthened process for identifying, reviewing, evaluating and integrating evidence to identify causes of human cancer. The advancements adopted include strengthening of systematic review methodologies; greater emphasis on mechanistic evidence, based on key characteristics of carcinogens; greater consideration of quality and informativeness in the critical evaluation of epidemiological studies, including their exposure assessment methods; improved harmonization of evaluation criteria for the different evidence streams; and a single-step process of integrating evidence on cancer in humans, cancer in experimental animals and mechanisms for reaching overall evaluations. In all, the updated Preamble underpins a stronger and more transparent method for the identification of carcinogenic hazards, the essential first step in cancer prevention. (shrink)

The HOD Dichotomy Theorem states that if there is an extendible cardinal, δ, then either HOD is “close” to V or HOD is “far” from V. The question is whether the future will lead to the first or the second side of the dichotomy. Is HOD “close” to V, or “far” from V? There is a program aimed at establishing the first alternative—the “close” side of the HOD Dichotomy. This is the program of inner model theory. In recent years the (...) third author has provided evidence that there is an ultimate inner model—Ultimate-L—and he has isolated a natural conjecture associated with the model—the Ultimate-L Conjecture. This conjecture implies that that the first alternative holds—HOD is “close” to V. This is the future in which pattern prevails. In this paper we introduce a very different program, one aimed at establishing the second alternative—the “far” side of the HOD Dichotomy. This is the program of large cardinals beyond choice. Kunen famously showed that if AC holds then there cannot be a Reinhardt cardinal. It has remained open whether Reinhardt cardinals are consistent in ZF alone. It turns out that there is an entire hierarchy of choiceless large cardinals of which Reinhardt cardinals are only the beginning, and, surprisingly, this hierarchy appears to be highly ordered and amenable to systematic investigation, as we shall show in this paper. The point is that if these choiceless large cardinals are consistent then the Ultimate-L Conjecture must fail. This is the future where chaos prevails. (shrink)

C. L. Hardin led a recent development in the philosophical literature on color in which research from visual science is used to argue that colors are not properties of physical objects, but rather are mental processes. I defend J. J. C. Smart's physicalism, which claims that colors are physical properties of objects, against this attack. Assuming that every object has a single veridical (that is, nonillusory) color, it seems that physicalism must give a specification of veridical color in terms natural (...) to physics, independently of our interests. Hardin argues that since physicalism doesn't give us any such specification of veridical color, this view is false. However, this argument assumes a mistaken account of veridical color. I show physicalism can appeal to an alternative account, according to which veridical color is characterized in terms of favored conditions of perceptual access, independently of any specification of the physical nature of color. With the success of cognitive science's interdisciplinary approach to studying the mind, many theorists have taken up the strategy of appealing to science to address long standing disputes about metaphysics and the mind. In a recent case in point, philosophers and psychologists, including Robert Kane, Daniel C. Dennett, and Daniel M. Wegner, are exploring how science can be brought to bear on the debate about the problem of free will. I attempt to clarify the current debate by considering how empirical (...) research can be useful. I argue that empirical findings don't apply to one basic dimension of the problem, namely the dispute between compatibilism and incompatibilism. However, I show that empirical research can provide constraints in connection with another fundamental dimension, namely the dispute between libertarianism, which claims that indeterminacy is, in certain contexts, sufficient for freedom, and hard determinism and compatibilism, which deny this. I argue that the source of the most powerful constraint is psychological research into the accuracy of introspection. (shrink)

I examine the metaphysical issue of the nature of color. I argue that there are two distinct ranges of colors, namely, physical colors, which are disjunctive monadic physical properties of physical objects, and mental colors, which are properties of neural processes. ;A pair of claims provide the motivation for subjectivist and dispositionalist proposals about the nature of color, proposals which I reject. The first claim holds that a description of colors according to our ordinary experience of color provides a specification (...) of some aspects of the nature of color. The second holds that our ordinary experience of color provides access to the nature of color. ;In chapter 1, I argue that visual experiences have mental colors, neural properties which importantly determine our color categories. However, I reject C. L. Hardin's and James A. McGilvray's arguments for the subjectivist claim that the colors we attribute to physical objects are mental colors. ;In chapter 2, I show that Paul A. Boghossian and J. David Velleman's arguments in support of subjectivism rely on the assumption that visual experience provides unqualified access to the nature of color. I argue, however, that objections to their projectivism about color perception provide strong reasons to reject subjectivism as well as their assumption that visual experience provides unqualified access to the nature of color. ;In chapters 3 and 4, I examine Mark Johnston's and Christopher Peacocke's dispositionalist proposals about the nature of physical colors, which are founded on the claim that ordinary visual experience provides access to an aspect of the nature of color. I undercut dispositionalism by rejecting their arguments for the claim that ordinary visual experience provides such access. ;In chapter 5, I show that Evan Thompson's proposal that the colors of objects are relations between properties of perceivers and objects assumes that a description of color according to our ordinary experience specifies some aspects of the nature of color. I reject this assumption by distinguishing between mental color and physical color. I conclude that rather than specifying the nature of color, descriptions of colors according to our ordinary experience merely serve to fix the reference of color terms. (shrink)

If I lead a life of virtue, that may well be good for you. But will it also be good for me? The idea that it will—or even must—is an ancient one, and its appeal runs deep. For if this idea is correct then we can provide everyone with a good reason—arguably the best reason—for being virtuous. However, for all the effort which has been invested in defending the idea, by some of the best minds in the history of philosophy, (...) it remains unproven. Worse, in this skeptical age hardly anyone really believes it. I don't really believe it either, at least not in its strongest forms, but I think that the question is nonetheless worth examining. Even if we cannot show that virtue and self-interest coincide, we can at least measure the breadth of the gap between them. (shrink)

Arbib's gestural-origins theory does not tell us why or how a subsequent switch to vocal language occurred, and shows no systematic concern with the signalling affordances or constraints of either medium. Our frame/content theory, in contrast, offers both a vocal origin in the invention of kinship terms in a baby-talk context and an explanation for the structure of the currently favored medium.
